Dr. Matthew Jenkins, an Internist with Carolina Medical Consultants, has been performing FAA physical exams for patients in the Rock Hill and Charlotte area since 2005. He performs Class I, II and III aviation exams along with Air Traffic Controller Exams. Dr. Jenkins is HIMS certified to assist in evaluating cases involving substance abuse or SSRI use.

All pilots are required to log in to Federal Aviation Administration MedXPress and fill out their portion of the 8500-8 form prior to their flight physical.

FAA MedXPress

This must be completed and a copy must be printed and brought to your appointment along with your confirmation code. This code is necessary to access your file online.

If you have been given a special issuance, please be sure to bring your special issuance letter along with any required documentation to your visit.

Key Points to Remember:

  • Please arrive 15 minutes prior to your appointment time to complete paperwork.
  • If you wear contact lenses, please remove them prior to your appointment and bring them with you so that we can check visual acuity with and without correction.
    • If you wear glasses, please bring them with you to the visit.
  • We do not file any insurance for aviation related visits.
    • There may be an additional fee for obtaining and reviewing outside information required to determine medical eligibility after your visit.
    • For a Class I EKG, there is an additional charge.
  • Payment is required at time of service

ATCS Exams:

  • Make sure you have Form 3930-4 with you or the appointment will be rescheduled.

HIMS Visits:

  • Please bring all required documentation with you to your visit
  • Complete and submit a release of information document 8065-2 to the FAA
